1. Application and Fee

  • Parents please complete the application and return it with a $50.00 non-refundable fee for each application submitted made payable to:

2. Admissions Exam (for Freshman applicants)

  • High school placement test - January 23, 2009 at 7:45 am at MDCHS
  • Freshmen applicants are required to take a standardized “High School Placement Test” (Scholastic Testing Service).

    Transfer student placement exam (for Sophomore & Junior applicants)

3. Interview

  • All incoming freshman must attend Freshman Interview Night, TBA
  • Sophomore and Junior Applicants will have a personal interview with a school administrator or faculty member immediately after the transfer student placement exam.

 

4. Request for Transcripts

  • Please Deliver the form directly to the student’s current school.
  • A transcript of grades and copies of standardized testing for the past two grades, as well as immunization records, will be needed for the application process.
  • Transcripts and records are to be mailed from the current school. MDCHS will not accept transcripts submitted by the parent or student.

 

5. Letter of Recommendation

  • All applicants must obtain a letter of recommendation from either a school administrator, teacher, or pastor.
